This information will be submitted directly to Maclay Track Boosters (Doug Covert as of this writing) and will not be shared with anyone other than Boosters members via the contact info link in the Boosters area of the website.  The information will be used to prepare and maintain an e-mail list for dissemination of important team and Boosters news and information.  Each e-mail distribution will have an opt-out link in the event that your student-athlete is no longer on the team or you do not wish to receive Boosters announcements for whatever reason.  When you click the Submit button, your default e-mail program will launch: just click the Send button and the information that you enter will be sent to me.  If your web browser is not set up to use a separate e-mail program, please send the requested information via whatever e-mail method you typically use to dugco@comcast.net.
